[BIP-365] Migrate Affected Composable Stable Pools Wave 1

Summary:
This proposal will migrate several gauges affected by the fee collection issue mentioned here. The gauges encompassed in this proposal will be migrated to new Composabel Stable Pools (version 5) and kill the gauges of the earlier versions that are exposed to the fee collection problem mentioned. These pools are critical to Balancer’s core liquidity and is why they will be migrated in this first wave. An additional wave will follow next week for additional. These pools were all core pools in their prior version, and will be so in their new implementation. The new gauges will be added upon this proposal passing (ASAP) and the gauges to be killed will be killed the week of July 31st for the new pool incentives to fully be established by the BIP-19 flywheel.
Motivation:
By migrating to these new pools Balancer LP’s are in a safe position while protocol fees can still be collected by the DAO. While no funds are at risk in the current state, the migration will once again enable the core pool cycle and protocol fee collection.

The Balancer Maxi LM Multisig eth:0xc38c5f97B34E175FFd35407fc91a937300E33860 will interact with the GaugeAdderv4 at 0x5DbAd78818D4c8958EfF2d5b95b28385A22113Cd and call the addGauge function with the following addresses as the gauge(address) and their network respectively passed as a string to gaugeType(string).
